2014-02-17 Database Error after upgrading Moodle / Questionnaire to 2.6.x Moodle Fixed by manually creating two tables questionnaire_fb_sections and questionnaire_feedback in MySQL database in accordance with mod/questionnaire/db/upgrade.php.
